Give the shot a stable identifier. Record which version is being reviewed.

What changes for the audience? What is known before and after this shot?

Describe composition, movement, the starting state, and the ending state.

Screen direction, eyeline, props, wardrobe, lighting, and scene geography.

List references, tool and settings actually used, and the person responsible for input permissions.

Keep, trim, replace, repair, or rethink? Record the issue, owner, and next version.
